24/7 Non-Acute Clinical Monitoring
Around-the-clock clinical observation under Supervising Physician direction and Registered Nurse care plan execution.

Managing Multi-Morbidities in a Safe Residential Setting
Many post-acute patients present with multiple intersecting chronic conditions. Our clinical protocols ensure smooth disease stabilization:
Strict scheduling and escort for Monday-Wednesday-Friday or Tuesday-Thursday-Saturday dialysis shifts, post-dialysis recovery rest, and fistula site monitoring.
Partnership with visiting Home Health infusion nurses for dressing changes, line flushes, and IV antibiotic bag infusion monitoring.
Continuous monitoring of room-air or low-flow O2 concentrators, daily SpO2 pulse oximetry tracking, CPAP/BiPAP adherence, and backup generator power.
Daily output logging, bag emptying assistance, skin barrier protection, and hygiene maintenance under RN oversight.
Clinical Examination Suite & Medication Station
Equipped clinical exam station with diagnostic monitors, double-locked medication cart, and sterile dressing staging table.
IJL Homes operates as a licensed residential medical respite facility. While our team provides 24/7 clinical observation and vital tracking, patients requiring continuous ICU monitoring or emergency medical resuscitation are directed immediately to 911 emergency services.
Clinical Inclusion & Exclusion Criteria
To ensure patient safety and maintain compliance with Title 22 residential respite licensing, we adhere to strict acuity thresholds.
Appropriate for IJL Homes Placement
Inclusion Criteria (CalAIM H0043)- Post-surgical convalescence requiring wound monitoring and dressing changes (Stage 1-2)
- Completion of IV antibiotic therapy via PICC / midline with home health coordination
- Fracture or orthopedic recovery requiring physical therapy and transfer assistance
- Chronic disease stabilization (Diabetes, Hypertension, Congestive Heart Failure, COPD)
- Dialysis-dependent patients (MWF or TTS) requiring escorted transit coordination
- Foley catheter or stable ostomy care requiring routine hygiene support
- Active enrollment or eligibility for Medi-Cal Managed Care (CalAIM Community Supports)
- Experiencing homelessness, unhoused living, or housing instability that threatens recovery
- Able to perform basic ADLs independently or with minimal 1-person assistance
Requires Higher Level of Care
Exclusionary Criteria (Requires SNF or Inpatient)- Bed-bound individuals requiring 2-person mechanical Hoyer lifts
- Active acute medical detoxification or unmanaged delirium tremens
- Active acute psychosis, violent behavior, or active suicidal/homicidal ideation
- Active untreated infectious pulmonary tuberculosis (requiring airborne isolation)
- Continuous IV medication titration or telemetry monitoring requirements
- Total parenteral nutrition (TPN) requiring intensive central line infusion monitoring
- Severe cognitive impairment / wandering risk requiring locked memory care unit
